LIABILITY WAIVER & RELEASE OF CLAIMS

Assumption of Risk

I understand that participation in activities at Happy Crab Play Center includes, but is not limited to, active play, climbing, jumping, use of play structures, party activities, character appearances, and interaction with other children.

I acknowledge that these activities involve inherent risks, including but not limited to injury, illness, or accidents that may occur despite reasonable supervision and safety precautions.

I knowingly and voluntarily assume all risks, both known and unknown, associated with my child’s participation.

Release of Liability

In consideration for my child being permitted to participate in activities at Happy Crab Play Center, I hereby release, waive, discharge, and hold harmless:

Happy Crab Play Center, its owners, operators, employees, contractors, volunteers, and representatives from any and all claims, demands, actions, or causes of action arising out of or related to:

  • Personal injury

  • Illness

  • Property damage

  • Loss or theft of personal belongings

This release applies to the fullest extent permitted by law, including claims arising from negligence.

Medical Authorization

I authorize Happy Crab Play Center staff to seek emergency medical treatment for my child if deemed necessary. I understand that all medical expenses incurred are my responsibility.

Health & Behavior Acknowledgment

I confirm that my child is in good health and able to safely participate. I understand that children must follow staff instructions and that unsafe behavior may result in removal from play areas.

Acknowledgment & Signature

I acknowledge that I have read and fully understand this waiver. I understand that by signing below, I am giving up certain legal rights, including the right to sue.
